Myocardial work index: a marker of left ventricular contractility in pressure‐ or volume overload‐induced heart failure

چکیده
Aims While global longitudinal strain (GLS) is considered to be a sensitive marker of left ventricular (LV) function, it significantly influenced by loading conditions. We hypothesized that myocardial work index (GMWI), novel LV may show better correlation with load-independent markers contractility in rat models pressure-induced or volume overload-induced heart failure. Methods and results Male Wistar rats underwent either transverse aortic constriction (TAC; n = 12) aortocaval fistula creation (ACF; 12), inducing pressure overload, respectively. Sham procedures were performed establish control groups (n 12/12). Echocardiographic loops obtained determine GLS GMWI. Pressure-volume analysis transient occlusion the inferior caval vein was carried out calculate preload recruitable stroke (PRSW), ‘gold-standard’ parameter contractility. Myocardial samples collected assess interstitial perivascular fibrosis area also atrial-type natriuretic peptide (ANP) brain-type (BNP) relative mRNA expression. Compared controls, substantially lower TAC group (−7.0 ± 2.8 vs. −14.5 2.5%; P < 0.001) only mildly reduced ACF (−13.2 2.4 −15.4 2.0%, 0.05). In contrast these findings, PRSW GMWI comparable sham (110 26 116 68 mmHg; 1687 275 mmHg% 1537 662 mmHg%; both NS), while found (58 14 111 40 1328 411 1934 308 mmHg%, 0.01). pooled population, (r 0.70; but not −0.23; 0.12) showed strong PRSW. correlated 0.61; 0.54; 0.001), ANP 0.85; BNP expression 0.75; demonstrated no marginal parameters. Conclusions Being conditions, reliable failure induced overload. reflects haemodynamic overload states, making more robust systolic should as an integrative marker, incorporating state, adverse tissue remodelling LV.

Left ventricular pressure-volume relationship in a rat model of advanced aging-associated heart failure.
Aging is associated with profound changes in the structure and function of the heart. A fundamental understanding of these processes, using relevant animal models, is required for effective prevention and treatment of cardiovascular disease in the elderly. Here, we studied cardiac performance in 4- to 5-mo-old (young) and 24- to 26-mo-old (old) Fischer 344 male rats using the Millar pressure-vo...
متن کاملLeft ventricular time-varying pressure-volume ratio in systole as an index of myocardial inotropism.
Time course of pressure/volume ratio of the left ventricle in systole has been proposed as a good index of its pumping properties; the ratio is an approximately steady function of time with its time course almost unaltered by changes in left ventricular end-diastolic volume (preload) and arterial loading conditions (afterload). It is the purpose of the present investigation to compare the ratio...
متن کاملValsartan, captopril, or both in myocardial infarction complicated by heart failure, left ventricular dysfunction, or both.
BACKGROUND Angiotensin-converting-enzyme (ACE) inhibitors such as captopril reduce mortality and cardiovascular morbidity among patients with myocardial infarction complicated by left ventricular systolic dysfunction, heart failure, or both. In a double-blind trial, we compared the effect of the angiotensin-receptor blocker valsartan, the ACE inhibitor captopril, and the combination of the two ...

SUMMARY The continuous pressure-volume relationships throughout the cardiac cycle were evaluated in children with tetralogy of Fallot, isolated ventricular septal defect, and patent ductus arteriosus. Biplane cineangiocardiography and simultaneous left ventricu-lar pressures were utilized for data acquisition. Normal pressure-volume loops demonstrated only small changes in left ventricular volu...

THE HYPOTHESIS that the physical work performed during ventricular systole is largely determined hy ventricular diastolic volume has had currency for the last 45 years." The oxygen cost of the heart's contraction has likewise been related to diastolic volume, and also to stroke work, mean arterial blood pressure times the heart rate, the integral of systolic pressure in time, and ventricular wa...
